HCV in Pregnancy

چکیده

Current US Centers for Disease Control and Prevention (CDC) screening guidelines (CDC, 1998); (Smith, 2012a) recommend that any woman—pregnant or otherwise—with a known or suspected risk factor, such as injection drug use or HIV infection, should be tested for HCV infection (see HCV Testing and Linkage to Care); (Koneru, 2016). Screening for HCV antibody should be followed by confirmatory nucleic acid testing (HCV RNA) for anyone positive on the screening assay (CDC, 2013) to differentiate past versus current, active infection. Women found to be HCV-infected should be linked to clinical care services for their infection(s) and for drug dependence treatment as needed. With current increases in HCV infection among young adults, including women of childbearing age, there is considerable discussion about the possibility of universal screening of pregnant women (Ly, 2017). Identifying HCV as women engage in prenatal care would allow appropriate assessment of liver disease status as well as establish care for their exposed children. This public health consideration will be weighed with concerns regarding potential cost and the logistics of linking patients to care.

Pathogenesis of Hepatitis C During Pregnancy and Childhood

The worldwide prevalence of HCV infection is between 1% and 8% in pregnant women and between 0.05% and 5% in children. Yet the pathogenesis of hepatitis C during pregnancy and in the neonatal period remains poorly understood. Mother-to-child transmission (MTCT), a leading cause of pediatric HCV infection, takes place at a rate of <10%. Factors that increase the risk of MTCT include high materna...

Hepatitis C virus in pregnancy.

Despite recent advances in the pathogenesis, treatment, and public health response to hepatitis C virus (HCV), HCV as it specifically relates to pregnancy has been a neglected condition. HCV-monoinfected pregnant women have a 2-8% risk of viral transmission to their infant, but the mechanism and timing of mother to child transmission (MTCT) are not fully understood, nor is the natural history o...

Increased Risk for Mother-to-Infant Transmission of Hepatitis C Virus Among Medicaid Recipients ― Wisconsin, 2011–2015

State surveillance during the last 10 years reveals a nationwide increase in hepatitis C virus (HCV) infection among young adults (1). The proportion of infants born to HCV-infected women is also increasing nationally (2). To estimate the proportion of infants born to HCV-infected women and the frequency of confirmed HCV infection in their infants, maternal name and date of birth from HCV repor...

Risk-Based Hepatitis C Screening in Pregnancy Is Less Reliable Than Universal Screening: A Retrospective Chart Review

Current guidelines recommend only hepatitis C virus (HCV) risk-based screening during pregnancy. We examined screening practices at a major medical center and found inconsistent risk-based screening and the presence of HCV among women with no known risk factors. We make a case for the implementation of universal HCV screening during pregnancy.

Intrahepatic Cholestasis of Pregnancy and Serum Bile Acids in HIV-Infected Pregnant Women

OBJECTIVES Intra-hepatic cholestasis of pregnancy (ICP) is uncommon, but has severe effects on pregnancy outcomes. ICP is characterized by elevated serum bile acids and liver enzymes and preferentially affects women with liver disorders. We compared bile acids and pregnancy outcomes of HIV-infected pregnant women, who commonly have elevated live enzymes, with uninfected controls. METHODS Twen...

Pregnancy outcomes associated with viral hepatitis.

The aim of this study was to examine the contribution of hepatitis B virus (HBV) and hepatitis C virus (HCV) to pregnancy-related complications including gestational diabetes mellitus (GDM), preterm birth (PTB), intrauterine growth restriction (IUGR), pre-eclampsia, antepartum haemorrhage and cholestasis. The Nationwide Inpatient Sample was queried for all pregnancy-related discharges, pregnanc...
